Cackwell and his partner Tuftridge are assigned to transport Kira Roskova to secure holding following her capture. The two agents ride in the reinforced prisoner compartment with Roskova, maintaining professional vigilance with weapons accessible but not drawn.

When the transport convoy is ambushed on an industrial access road by Icener's forces, Cackwell moves toward the cab to check on the driver as gunfire erupts. A flashbang detonates, incapacitating both agents. In the chaos, Roskova's restraints are cut and she escapes with her rescuers.

Both Cackwell and Tuftridge survive the ambush, though Cackwell sustains a serious shoulder wound that requires surgery. Director Costa receives the casualty report at the CCC: "Two wounded, one critical. Cackwell and Tuftridge both survived, but Cackwell is in surgery."

The ambush demonstrates that Icener's network had intelligence on ZSI transport routes, timing, and procedures---a significant security breach that escalates the crisis.
